A transvestite would NOT want breasts nor SRS.
A transgender would want breasts but NOT SRS. They would be squeamish about breasts because it would publicize their fetish. They also wouldn't want getting them to interfere with their ability to have orgasm with their penis. But having breasts would validate their external gender.
A transsexual would want breasts and SRS. Having breasts would not be uncomfortable nor make them squeamish even with losing ability to orgasm. They would universally prioritize having SRS over breasts. Having SRS would validate their internal sex.
