@clifford0584 It’s about familiarity innit? Anaesthetic training familiar with seldinger technique, perc trache and poking gently with needles. ED training used to scalpels, bigger cuts, get it done. Elective OT and ED are not the same thing. Anaesthetists rarely pick up scalpels. ED do.

@EmmaPNG See I think this is where the confusion lies. Anaesthetising 13 patients is a big day. We spend every moment making sure nothing goes wrong. The vigilance and fretting creates the ‘boring’. Boring means all went well. Boring is good. We strive for boring.

On days like today I have a little giggle remembering all the people over the years who have told me they couldn’t do anaesthesia because they think it would be so boring.

DAMMIT. I did say anecdotally there was a lot of Covid around in the SE. Feeling quite rough but last night's fever has eased. Working out arrangements to isolate from GCSE teen in the house are like a pro level farmer/fox/hen/grain problem, and I'm not at my cognitive best.

@cliffreid 1 ampule if <40kg. 2 ampules if 40-80kg. In adults I use 2 up to 80kg and 3 if >80kg because it is safe and who needs maths in emergencies.

What dose of rocuronium in mg/kg would you use for paediatric RSI in the emergency/crit care setting and WHY?

@draj79 Set up before hand to do as part of intubation. If just for the case orogastric. If RSI once ett confirmed place it before tying ett. If not do first. If needed mid case - all the other suggestions with cold NGT.

Covid caused a collapse of healthcare as we know it. Nearly every icu bed we had, was occupied by covid. We couldn’t operate, colleagues died, and long covid sequelae will dog healthcare for years to come. Testing was only way to control spread…forget this “scientist”

@EM_Dr_Jacklin They didn’t make it out the door to come back. First time at an arrest when everyone waited for me to finally call it. And as it was 20 years ago, I was then the sole Dr in ED for another 8 hours….it’s a tough speciality.

@GongGasGirl Good analgesia for some. Good vomit inducing hallucinations for others. Bit like adults really. 1mg/kg innit? Personally find it a better analgesic with less side effects than oxycodone. Had to wash the vomit out my toddler’s hair on it tho 🤷🏽‍♀️

@MStott88 I can now tolerate a certain Brian Adam’s song, still brings back the memories of a patient in 2002, but without the harrow. I would seek help. You went thru something very traumatic. It is normal to think on it, but not quite as you describe.

When One Last Time by Ariana Grande comes on the radio, I’m immediately taken back to the sights, sounds & smells of 22/5/17. I go over, step by step, the trauma laparotomy I performed as ST3. Nothing else, even her other songs, trigger this. Is this PTSD and should I seek help?

@AndyGuest73 @freshblooddr I do "Clow [pause] that's see ell owe double-you". Empirically, the pause is critical, and half the time I try "charlie lima oscar whisky" they get confused and think my given names start with Charlie. Still get Cloe, Chow, Chou, Clough, etc. My Dad got junk mail to "Dr Claw"!

I've been back in clinical work 3 weeks and I must say "like the animal" when spelling my name on the phone at least 15 times a day. How do *you* spell out your name?

I'm at the point where if you safe abusive things about OzSage, I'll block you. The science is solid, even if you didn't like predictions that were wrong early in 2020. I just don't want to hear it any more. Vaccine, ventilate, mask indoors. When you don't, you kill ppl.
