80s y/o patient previous SAVR Magna Ease 21 mm with severe degenerative AS. Prohibitive coronary obstruction risk on the LCC (LM height 1.3; VTC<2) Borderline on the RCC. Severe mixed MS/MR with severe MAC. Successful first UNICORN ViV at @AtlanticHealth, traversed with AL3-Finecross-Astato at 70W; balloon valvotomy with Emerge 4.0, switched to stiff 0.035 wire, followed by laceration with Armada 16 mm.
