John Snow, Nicolai Pirogoff, Jean Flourens, and Arthur Guedel described the different stages of anesthetic depth. But only Guedel's stages became clinically popular. Why? He added two useful signs that others had overlooked: eye position/eye movement and breathing pattern.

The difficulty with the midline approach to the thoracic epidural space is partly because the spinous processes are steeply angulated. The other problem is that the spaces between the spines are slightly curved (image: Moore - 1961).Thus a straight needle must slide off bone.

Ohtani was probably destined to be a Dodger. They had scouted him since high school, making an enormous effort to sign him back then. He was likely a Dodger fan growing up, and he probably would've signed with them in '18 if not for the DH rule. This talk that he left the Angels for a crosstown rival or "Trout is loyal and Ohtani isn't" is BS. For one thing, the Dodgers don't play in the same division as us and they're not even in the same league. The Angels and Dodgers aren't like the rivalry between the Dodgers and Giants or the Red Sox and Yankees. It's really only our owner, obsessed with market share, who sees the Dodgers as rivals. Maybe we can talk about loyalty if Ohtani had tried to engage a division rival like the Mariners or Rangers in a bidding war, but he didn't do that! We Angel fans should consider ourselves lucky for having him for six years when he was in his prime. His decision to leave was not a betrayal, but rather that of someone who, in his heart, probably always wanted to be a Dodger.
