NEWS FROM INDUSTRY: KEYNOTE-C93 Update in #EndometrialCancer Source: Merck press release The Phase 3 KEYNOTE-C93 trial showed pembrolizumab monotherapy significantly improved progression-free survival compared with platinum-based chemotherapy in patients with previously untreated advanced or recurrent endometrial cancer with mismatch repair deficient (dMMR) tumors. The study demonstrated meaningful response rates, complete responses, and durable responses, with a safety profile consistent with prior pembrolizumab studies. Standard post-operative platinum chemotherapy for LGSOC is a biological fossil, a high-grade protocol forced onto an endocrine-driven disease where response rates struggle to break 13%. We must stop defaulting to this “platinum sprint” and shape the field around the #GOG3026 results (ribociclib + letrozole), showing a 30.6% ORR and a 21.2-month DOR in first-line recurrence, prioritizing durability and quality of life upfront while reserving the high-pressure strike of MEK/FAK inhibition for second-line progression, where avutometinib + defactinib achieves a 44% ORR in KRAS-mutated cohorts. To truly move the needle, we must challenge the necessity of adjuvant platinum altogether, moving endocrine-targeted strategies into frontline maintenance or as a complete replacement for cytotoxic therapy post-surgery.
