If you're connecting at Heathrow or Manchester and staying airside (no passport control, no terminal change, no baggage reclaim), you're currently exempt. The Home Office has called that exemption temporary and under review.
Every other UK airport requires one for any transit.
Here's the variable most people skip. Your connection breaks. Flight delay. Cancellation. Forced terminal change. Now you're landside. Without an ETA, the airline won't reboard you. They check before you fly.
