@FridaRosenblum @sam_albadri There is no magic time. Typically a few daybut depending on the injury and sloughing of the endothelium thrombi can persist for longer especially if patient on CNI. Early AMR would be less favored of DSA is negative pretransplant and C4d is neg. Important to rule out ongoing TMA

In deceased donor kidneys with head trauma and fibrin thrombi, what is the timeline for them to disappear/wash out? What if you see endothelial swelling and other features of TMA PO day 6? Is that donor derived or early AMR? @VineetaKumar8 @JeanHouMD @AstridWeins @SethiRenalPath

@RajguptaEM Interestingly we found that glomerulitis and high proteinuria are associated with worse prognosis in primary MN in the allograft (your pt have both plus low albumin). The glomerulitis in this case may not reflect AMR but stasis and may need to rule out renal vein thrombosis

ESRD from HTN, DDRT >5 yrs back, presenting with new-onset nephrotic proteinuria. PLA2R+ (IF & serum). C4d-, DSA-. Several gloms show glomerulitis, doubl contours and mild mesangiolysis, ptc1. Is this de novo MN some form of unusual CA-AMR, or unrelated to Tx? #askrenalpath

@RajguptaEM Although no one will know for sure my impression is that AMR associated MGN is weak here for a few reasons: PLA2r is pos, DSA neg, C4d neg and ptc0-1 (none of these reasons by itself is definitive but the combination argues against this possibility). It is likely primary MN
