@fgomsan Agree 49% with pad use is abnormally high.. though w only 147 consecutive cases in 6 years perhaps due to volume/learning curves in the different surgeons.. probably would have been better to study this metric after several hundred had been done by a single high volume surgeon…

This paper relates membranous urethral length to postoperative stress urinary incontinence after HoLEP... It kind of blames the patient's anatomy for the occurrence of postoperative stress incontinence. I think with these incidences of post-op incontinence, the surgical technique used by the authors is a huge problem. Or is it that I only operate on patients with long urethral lengths to get way much lower rates of incontinence? (49,7% SUI at 1 month shows a flaw with surgical technique)

@Mohamedessam997 @soares_uro @MayoUrology Mid trigone injections are very safe. I save 1ml for injection (10 units) posterior to mid trigone and one injection anterior. The only notable finding is sometimes there is a bit of venous oozing which subsides with holding pressure using the cystoscope tip in the office

@jfunkmd @RomanCarvajal Same and I’ve noticed pieces of tissue are more likely to clog the blade by going through both openings than non-urolift cases. Nothing a quick dismantling of the blade and a sharp tap to dislodge the tissue can’t fix 👍🏽

@RomanCarvajal In the US we have disposable blades. I just chew through the tabs/staples. About 1 in 10 cases I have to use a second blade but mostly it is straight forward. 👍🏼

@amy_krambeck @NM_Urology @EndourologyNm Previously seeing this with some rezum cases but now seeing more and more of these post Aqua cases as the residual lateral lobe adenoma regrows or grows tissue bridges together in midline causing a tortuous urethra. Fortunately very straightforward HOLEP cases!

@justindubinmd Another article said he was having urinary symptoms. DRE was likely done for this reason, correctly so, which resulted in eventual mCSPC diagnosis. It is now up to us to educate the pending barrage of 80+yo elevated PSA/“prostate check” pts about symptomatic treatment

@AustenSlade @RomanCarvajal @lfredosanchez I incise bladder neck prophylactically at 3 and 9 to fat when <40gm and very large capacity bladders. Idea being weak bladders don’t keep the bladder neck open and it scars down. Have had 3 BNC prior to doing this and all were treated with a TURBNC with 80gm kenalog injection
