@tomgoom So for a true bone stress - if we are getting pain but it’s settling in 24-48hrs we should be looking to reduce the running volume down for the next one (once symptoms have settled) to try to avoid any pain at all occurring afterwards?

Yes, ideally pain free during and after including the next 24-48 hours. To be fair though there isn’t a great deal of research on this. There’s also a disparity between MRI findings with bone stress injuries and pain. So there may be times when it’s acceptable to work with some symptoms.

‘Shin Splints’ (Medial Tibial Stress Syndrome - MTSS) is different from other running injuries. It doesn’t respond well to continuing to run with pain. It’s considered a Bone Stress Injury and optimal loading for these is pain free both during and after. . In conditions like tendinopathy or patellofemoral we often suggest a traffic light system where pain up to 3 out of 10 is ok, providing it settles quickly after. This just doesn’t work for MTSS! . Ideally we find a pain free level of training but if we can’t then a rest from running, impact and painful activities can really help things settle and facilitate are more successful return to sport. . Shin pain is complex and there are multiple differential diagnoses to consider, including high risk stress fractures. I’ve been really careful but caught Covid three weeks ago. I’m pretty fit but ended up in hospital struggling to breath but was looked after incredibly well by @SalfordRoyalNHS and am recovering now. Please take my advice, continue to keep safe & don’t get it (it’s horrible).
