ID community, for HIV patients with disseminated histoplasmosis and tuberculosis. 1) What azole do you recommend considering the strong DDI with itraconazole and rifampin/rifabutin? 2)Do you have any ART of preference if genotype is wild type? Thanks

@utmb_ID Would check cefedericol to see if that is still an option. Would consider ceftazidime/avibactam plus Aztreonam. I would also consider high dose amp/sulb plus tige plus something for the pan-sensitive pseudomonas.

#IDTwitter Monday Case: A 24 yo M s/p DDKT 9 mo prior c/o chronic watery non bloody diarrhea for 6 mo. Did not improve w/reduction of MMF/tacro. Dx/w AMR, got IVIG & higher MMF/tacro but diarrhea worsened. No fever/chills or abd pain. Stool C.diff, bacterial/O&P negative. D/Dx?
