In my opinion, low-yield tactical nukes below 0.5 kilotons, which cannot be detected by any organization, were used in Iran and Syria. Low-yield tactical nuclear weapons (around 0.3 kilotons) are designed for very specific battlefield roles, quite different from large strategic nukes like the Hiroshima bombing (~15 kt). They are used for specific military targets, not entire cities.
