You’re right that medical-school debt is a major pressure point. Make public med school free or heavily subsidized would increase applications. But if the goal is more primary care access, fewer corporate bottlenecks, and lower costs, then expanding full practice authority (FPA) for nurse practitioners (NPs) also has to be part of the solution. HRSA shortage-area formulas still focus on physician ratios and do not fully account for primary-care services already being delivered by NPs and PAs. Evidence shows NP-led primary care can expand access without sacrificing quality or raising costs, especially in Medicaid and underserved populations. So yes, your suggestion would reduce physician debt. But also let NPs practice to the full extent of their education and training. The primary care gap is too large to solve with physicians alone. This is not MD vs. NP. It is access vs. artificial scarcity. I’d also argue that if physician-owned hospitals can be restricted, then private equity ownership of hospitals can be restricted as well. PE firms buying up smaller hospitals, increasing costs, cutting services, and relying on aggressive Medicare/Medicaid billing strategies to maintain profitability is not a sustainable long-term healthcare model. At some point, the business model undermines the care model. The trust between clinicians and corporate healthcare is already fractured. Patients feel it, providers feel it, and communities feel it. When access, staffing, and treatment decisions appear driven by margins rather than clinical need, trust collapses for everyone, except perhaps the executives, major donors, and insiders who can still obtain preferential access. Healthcare cannot function well when the people delivering care no longer trust the people controlling the system. You have seen behind the curtain and have experienced it yourself. How do we fix it? Make healthcare a “societal good” rather than an exploitable business model?

If you want more doctors doing house calls, not selling their practices and going to work for the big HC conglomerates, make public med school free. A little gov intervention, so that rather than having 100s of thousands in debt guiding their decisions , they can do primary care or be a family physician and spend as much time with patients as they want. They can take cash. They can take chickens. If you had 250k after almost a decade of school, do you think that would impact your decisions ? And if you own a big HC conglomerate, does knowing they are drowning in debt impact your decision and how you compete and contract with them ? Fuck yeah it does. You pressure them till they have to sell out to you in an acquihire. They can’t afford to survive on their own and every huge HC company takes advantage of them About 32k students enter med school and DO school a year. 75k for a grant each. Thats 2.4b annually for each class. That’s it. You want better healthcare for everyone. That’s the place to start.

Elon Musk pense qu'on est dans une simulation. Moi, mon film préféré c'est Matrix. Et voici ma théorie sur la simulation : C'est improvable, ok. Mais en tant que philosophie de vie, c'est redoutable. Le principe : pour que la simulation reste active, il faut qu'il y ait du fun dedans. Sinon les designers s'ennuient et ils débranchent. Or, qu'est-ce qui tue le fun ? Les systèmes rigides. La bureaucratie. Quand tu enlèves les libertés individuelles, tu tends mécaniquement vers un système boring. Des formulaires, des comités, des normes, des sous-comités sur les normes. Plus personne ne crée, plus personne ne prend de risque, plus personne ne joue. Et là, les mecs derrière l'écran regardent leur dashboard et se disent : "bon, faut faire quelque chose." J'ai vu passer une théorie qui m'a fait mourir de rire : le Covid aurait été envoyé par les designers exprès. Pas pour nous nuire — pour pousser la bureaucratie le plus loin possible. La forcer à se révéler dans toute son absurdité. Confinements, QR codes, autorisations de sortie, comités d'experts qui se contredisent en boucle. Un stress test à l'échelle planétaire. Le but : faire péter le système par excès, pour permettre le reset. Et c'est exactement ce qu'on est en train de vivre. Trump, Musk, Milei — ce sont les incarnations du patch. DOGE qui démantèle les agences fédérales. Milei qui tronçonne l'État argentin en direct. La tech qui reprend le narratif. Le retour brutal des libertés individuelles comme valeur centrale. On assiste à un renouveau de civilisation. Et il est massivement basé sur la liberté de l'individu de créer, de buildre, de prendre des risques. Conclusion opérationnelle : Traitez la vie comme un jeu vidéo. Accumulez un maximum de skills. Buildez des trucs. Faites des choses qui vous donnent du fun, ou qui donnent du fun à l'humanité — et accessoirement, aux types qui nous regardent depuis l'autre côté de l'écran. Soyez intéressants à regarder. C'est littéralement votre seule mission.
