I’ll never forget the week hospital admin brought in some fancy speaker from the Minneapolis VA to teach “Collaborative Note Taking” A.K.A. writing the session note together with the patient during the therapy hour. I asked: “but what if they came for therapy?”

“In order to practice with cultural competency in this area…” What ever happened to plain old competency? Plain old competency takes account of unconscious mental life: we do not fully know our own hearts and minds, there are multiple layers of experience, and emotional life is complex and often contradictory. Plain old competency means working to understand our patients as individuals, and helping them come to know themselves more fully and deeply. Moral posturing is not a substitute for clinical competence. Moral posturing is incompatible with developing clinical competence.

Skilled therapists don’t build “rapport” by talking about themselves. It’s a beginner’s mistake. Non-therapists (and poorly-trained therapists) bring a frame of reference that applies to *social* relationships, not therapy relationships.
