Calling Cuba’s doctor-export program a “gift to humanity” ignores how it actually works. It is run by the Partido Comunista de Cuba as a business model, not as charity. First, it is not a gift. Host countries pay the Cuban government for those doctors, and the doctors receive only a small fraction of that money. The state keeps the rest. That makes it a labor-export industry, not humanitarian aid. Second, it is not truly voluntary. Doctors cannot freely negotiate contracts, their movements are restricted, and quitting or defecting can lead to punishment or family retaliation. A system based on coercion cannot honestly be called humanitarian. Third, it harms Cuba’s own people. By exporting doctors for revenue, the government leaves hospitals in Cuba understaffed and short of specialists, while ordinary Cubans face long waits and lack of care. A government that claims to serve humanity should not do so by weakening healthcare at home. Fourth, countries that accept these doctors are participating in exploitation. This is no different in principle from exploiting illegal immigrants for cheap labor: they get workers at a discount, ignore the conditions those workers live under, and justify it by saying “at least they have jobs.” In both cases, the system works because the workers lack real freedom and bargaining power. So the honest description is this: It is not a gift to humanity. It is a state-run labor export scheme that exploits doctors, weakens Cuba’s healthcare system, and allows foreign governments to benefit from coerced, underpaid labor while pretending it is solidarity.

For decades, Cuba has been sending thousands of doctors to respond to health crises across the world. But the U.S. is trying to put a stop to that – a move that could have far-reaching impacts across the Global South.
