First, there should be informed consent. Second, current package inserts for all Covid mRNA vaccines up till today state certain things like carcinogenicity have not been studied, and so this should be part of informed consent. Third, if certain outcomes such as cancer have been associated temporally and spatially with Covid mRNA vaccines, that should certainly be included even in the absence of established causality as that may take many years to resolve or may remain controversial. That can be shared as well. Fourth, the physician or other health care individual obtaining consent could state that the exact risk of cancer is likely low but that some people may be susceptible to cancer due to differences between individuals including variations in the immune system, or other genetic or environmental factors. Fifth, informed consent should mention that Covid mRNA vaccines increase PD-L1 expression in tumors if a person has cancer. PD-L1 is an immune evasion mechanism that is cancer-promoting. Sixth, informed consent should mention the availability of Covid peptide mRNA vaccines as an alternative. Seventh, informed consent should include current risks associated with Covid infection such as risk of intubation vs asymptomatic infection and risk of death. Eighth, informed consent should mention that long-term outcomes including risk of cancer have not been studied in individuals who have had 5, 6, or 7 shots. Ninth, informed consent should mention that some tumors have been reported to contain spike protein expression with absence of nucleocapsid protein consistent with Covid vaccine vs Covid infection. Tumors are not expected to express Spike protein and while the numbers are low for such reports, they do raise concern as tumors should not express a foreign protein such as Spike which may have unknown or unproven consequences with regard to tumor growth or its response to anti-cancer treatment.
