The annual UNC Cardiology Faculty-Fellows basketball game lived up to all its hype! The faculty has decided not to publicize the final result. No need to bruise those youngsters' egos. The MVP earned an extra week of CICU nights! @UNC_SOM @UNCDeptMedicine @UNC_Health_Care

Lots of questions about the Covid-19 vaccines -- here are some of the more common ones, with my answers. Directed at clinicians, but probably something here for all. Any that we missed? @NEJM nejm.org/covid-vaccine/…

@MaxDiddams 1/2 Worsening hyperkalemia on CV can only be because of a few things. Is it not getting cleared? Or is it getting produced/retained? Or testing error? Would think access issue, hemolysis (traumatic or allergic), high cellular turnover (rhabdo, tls, etc), hematoma reabsorption.

Ever had a patient with rising K on CRRT? Checked the usual suspects and absolutely stumped. @NephroPeeps

#JACCINT #AHA20 research: NRD data shows an increase in proportion of PPM implants during a subsequent hospitalization after discharge among patients who underwent #TAVR between 2012 and 2017, as length of stay for index TAVR trended down. bit.ly/3k7leco @MazzellaMD

Excited to share three new ICU #OnePagers on a couple important critical care topics: acid base, arterial lines, and EVDs. Lots of nerdy fun here including Winter's formula, Lundberg waves, and pulsus alternans. Also an updated version of the ARDS OnePager. #FOAMed #FOAMcc

My ratio of positive evaluations* of medical students and residents to negative is about 9:1. *Above average to downright glowing Nobody told me how to write evaluations of learners. Come along while I discuss what I have learned. 1/

All my papers are late. All my emails go unanswered until a second poke, except a tiny random sample that I answer within one hot second. All birthday wishes are belated. My grants go in late or not at all. Workouts have gone out the window. So have quotas on chocolate.

4. In myeloproliferative diseases, extreme thrombocytosis may increase bleeding risk due to acquired von Willebrand disease. Any other examples you can think of?

Medicine is fascinating because things are not always as they appear on the labs: 1. Increased risk of clot despite thrombocytopenia in HIT 2. In APS, will often see an elevated baseline aptt despite higher risk of clot 3. In cirrhotics may be hypercoagulable despite ⬆️ INR

Today, I met the people who saved my life. I’ve been tweeting about my cardiac arrest since I woke up in the ICU and re-downloaded twitter (@LGlaucomflecken deleted it for my own safety and concluded that if I could figure out how to get it back, I was safe to tweet again)
