Homeless people are using dogs to test drugs for them on Skid Row in Los Angeles, California. Non-profit group Starts With One Today says dogs are being abused, thrown away, sold for drugs, neglected, and fed drugs to make sure there is no fentanyl. Animal advocates are ripping Mayor Karen Bass and other elected officials for not taking enough action. “We’re coming out here, risking our lives to help these dogs with no support from the government,” said volunteer Joey Tuccio.

His op note discusses a trad blunt maneuver to elevate the spleen. He clearly confused liver edge for spleen edge and blindly cross clamped the porta hepatis, cut, and GAME OVER

The OR nurses would have no idea what was going on most likely & aside from a large clamp he put across the porta hepatis (thinking it was the spleen hilum) there were likely no special tools

This is not just a surgeon problem. A competent nurse (RN) should have stopped the doctor from performing a procedure not included in the informed consent.
