Four years, people have sent me messages where they get a diagnosis from their doctor, usually something orthopedic in nature But the physician gave them no explanation as to what to do about it, how it developed, why it happened Often my explanation to them is simply a layman's terms rehash and general suggestion to see a specialist or how to approach rehab I'm not calling all doctors lousy, but clearly there is a deficit in bedside manner and patient communication amongst many

@AJA_Cortes As a chiropractor ive seen typical flow for allopathic mgmt of spinal ortho conditions. Muscle relaxers first, PT second, MRI and surgery or pain management next. Joints need to move to survive. PT is great but doesn't address joint motion. Chiropractic needs to be first line

A patient with leg pain to the calf performs repeated extension exercises and the leg pain appears to migrates proximally to his buttock. On stopping the pain does not return to the leg, but he reports tingling in his foot. Is this a centralisation response?

@mikereinold The way I learned it was, DPT shows the academic degree and PT shows the license. If I let my license lapse, I believe I could still use DPT in my name, but not PT. Not sure the validity of that, but it's what I remember.

Honest question for my PT friends out there - why do we put the initials "PT, DPT" after our names? Why the repetition? I get it, we are PTs, and you have a doctorate degree. Why not just put "DPT?"

Macrohard or Digital Optimus is a joint xAI-Tesla project, coming as part of Tesla’s investment agreement with xAI. Grok is the master conductor/navigator with deep understanding of the world to direct digital Optimus, which is processing and actioning the past 5 secs of real-time computer screen video and keyboard/mouse actions. Grok is like a much more advanced and sophisticated version of turn-by-turn navigation software. You can think of it as Digital Optimus AI being System 1 (instinctive part of the mind) and Grok being System 2. (thinking part of the mind). This will run very competitively on the super low cost Tesla AI4 ($650) paired with relatively frugal use of the much more expensive xAI Nvidia hardware. And it will be the only real-time smart AI system. This is a big deal. In principle, it is capable of emulating the function of entire companies. That is why the program is called MACROHARD, a funny reference to Microsoft. No other company can yet do this.

@IdleGlands @TeslaCharging It only defrosts the windshield and heats the cabin, but doesn’t heat the battery to optimal driving condition. I always have to drive with a cold battery degenerative after staying parked for 30 minutes where there is no charging at that place in winter.

@DanLairdMD Was pain the only symptom? I'm not an MD but a 'crushed spinal cord' would usually present with other issues that would involve immediate intervention to prevent potential paralysis, incontinence, permanent gait disturbance etc. If the patient was told to leave I'd question ER.

Listen, I’m gonna say what every woman with any sense is thinking but is too polite to mention. Let me tell you something about MEN. Not boys. Not “finding themselves” philosophers with ideas of grandeur to rebuild rural America with a baby on the way. You wanna talk about “simple living”? About finding some $80,000 shack in the middle of nowhere and calling it financial freedom? Cute. Real cute. You know what’s NOT cute? Having a newborn baby and wondering if you can afford formula and food on the table this week because daddy decided his self-actualization journey was more important than building generational wealth. I know MEN. Real men. The kind who get up at 4 AM and come home at 7 PM because they understand that providing isn’t optional… it’s the FOUNDATION. These are the men who built this country. Brick by brick, deal by deal, sacrifice by sacrifice. My father worked doubles. My uncles worked doubles. Every man worth knowing in my life has understood one fundamental truth: when you have a family, you don’t GET to be tired. You don’t GET to float down rivers contemplating your journey. You PROVIDE. Period. They did everything in their power to make sure their wives didn’t have to choose between the electric bill and groceries. You know what I see now? Boys convincing themselves poverty is actually minimalism. That failure is actually “rejecting the system”. There’s a difference between struggling despite breaking your back every day, and struggling because you’ve decided “simple living” means checking out of the rat race before you ever really entered it. One is noble. The other is just… lazy with a Pinterest aesthetic. And before anyone comes at me with “but mental health” and “but work-life balance”, FUCKING SPARE ME. You think my father didn’t want to relax? You think the men in my family wouldn’t have LOVED to take a Tuesday off to go fishing? But they had CHILDREN. They had RESPONSIBILITIES. They understood that being a provider isn’t about finding the path of least resistance. This is the season you GRIND. Where you work yourself to the bone. This is when you sacrifice EVERYTHING to build the foundation your child will STAND ON. These young guys with their manifestos about rural life and affordable housing. Fine. Beautiful vision. Then what? You moved, congratulations. Where’s the DRIVE? Where’s the HUNGER?? BUILD SOMETHING. You just brought a CHILD into this world, and your priority is… simplicity? Your baby doesn’t need simple. Your baby needs OPPORTUNITY. Security. Options. These same guys will preach about “masculinity” and “traditional values.” Brother, traditional masculinity IS providing. Traditional values MEAN you sacrifice your comfort for your family’s security. You don’t get to cherry-pick the aesthetics and skip the substance. I’m not saying everyone needs to be a Wall Street shark or a Silicon Valley tech bro. But there’s a middle ground between selling your soul and floating down a river while your pregnant wife watches you “find yourself.” The men I respect? They’re BUILDING something. Even if they’re not millionaires, they’re STRIVING. They have AMBITION. They see their children as motivation, not as accessories to their homesteading fantasy. Every real man I know treats fatherhood like war. You show up. You dominate. You WIN. Because your child deserves better than your surrender into poverty cosplay for virtue. FRANKLY, I’m disgusted you would treat building a family in such a LACKADAISICAL manner. Grow up.
