On my last clinic day of 2025, I am DOD for SBRT, serving as the physician for the thoracic consultation clinic and covering emergency Radiation Oncology at MD Anderson. Holidays are always very challenging for our patients because cancer doesn’t take a vacation. From my 23 years of practice, I have learned that my best teachers are my patients. They have taught me how to maintain hope in the face of a "non-curable" diagnosis. When they supported my hesitation with trusting eyes and clasped hands, I felt re-energized. And when I could report "No evidence of disease," the joy radiating from their faces gave me the most profound understanding of life’s value and success. Happy New Year to you all!

@DavidSherMD Proton therapy at greatly reduced cost is right around the corner.

All that being said, should value influence whether a therapy is a standard-of-care? As a general principle, I think it should, although in the US that has never been a consideration. It is also quite difficult to establish our societal willingness-to-pay threshold. Radiotherapy is profoundly, profoundly undervalued in the US, so why should we as a field handicap ourselves relative to our peers? Historically, the difference between IMRT and 3D and between hypofractionated and conventional RT almost always favored the more novel treatment from a cost-effective perspective. It was great! However, as the gains with more expensive technology become increasingly marginal (and I realize there are different takes on this paper that would say it’s absolutely NOT marginal... but wait for TORPEdO!), we have to have these conversations.

There have been several thoughtful and important comments about cost-effectiveness of IMPT, both in the US and internationally. We had looked at the cost-effectiveness of IMPT in a prior publication back in 2018, and while the costing environment may be different today, I think there are several lessons from the analysis. pubmed.ncbi.nlm.nih.gov/29976499/
