When I was a medical student, I vividly recall taking care of very very sick patients with acute pancreatitis in the ICU. Remarkable to see how APOC3 inhibition (here, olezarsen) is now shown to be a highly effective therapy for severe hypertriglyceridemia, esp among those with prior pancreatitis to reduce risk for acute pancreatitis (NNT 20 for all and 4 for those with prior) & yield remission (up to ~50%) for hypertriglyceridemia. Here you go, this represents hundreds of thousands of study subjects in rigorous well designed trials. Ference et al Eur Heart J 2017. Everyone can plot their own "average" risk on this graph. 1 mmol/L = 38.67, so 2.5 mmol/L is about 100 mg/dL. Bottom line; risk for CVD at population level (not individual level per se) is proportional to "walking around" LDL-C, but risk is higher if you had a prior event. Of course the issue is one cant predict risk with certainty at individual level. If we could then we would not need to treat so many people. Maybe some day we will be better able to define risk at individual level to "personalize" therapies, but I suspect not in next 1-2 decades.

A great listen where @NutritionMadeS3 discusses the recent keto Trial. He interviewed the only expert whose name is on the paper and it seems as if Dr Buddoff had never read or signed off on the paper written by a lipid-untrained citizen scientist and medical student. He seemed to be making up answers on the fly. Gil's analysis was spot on, and all should listen to the last 4-5 minutes with Gil's concluding thoughts. I do not see how JACC is ever going to accept even a rewritten attempt. As Gil noted the LMHR crowd has long endangered public health. Time to stop.
