@nikillinit + Carbon's new deal with BCBS of Mass. is risk-based for commercial. Myoung Cha confirmed this in an Axios event yesterday and also predicted on this site that VBC in Commercial will take off in 2023.

@MatzkinHealth 100% agree. New (sometimes recurring) revenue stream. I do worry about the incentives, though. Telehealth loss leader + pharmacy "SaaS" revenue is a recipe for clinical disaster, particularly in controlled substances.

"Virtual care" is so much mroe than virtual visits. In fact, I'd argue the best virtual care companies are actually more value-based care companies than virtual care companies. To build a real VBC model, we need to go *way* beyond virtual visits and incorporate many more tools

@sacjai @forbeshealth Great opportunity here for highly accurate, patient-specific predictive analytics. Remove ambiguity about risk, complexity, and right level of care for individual patients as early in an episode as possible (or, better yet, proactively). @verilylifesci is working towards this.

As always, the disruptive innovations (e.g., virtual clinics, teletherapy, etc.) will gradually get better, move upstream, and disrupt traditional models. In the long run, traditional care providers will have to choose: Adapt or Be Disrupted. Some sooner than others... 4/4

2) Disruptive Innovation: Deploy new care models (e.g,. @onduo) to supplement traditional practice, or replace select care with new, "good enough" models (e.g., @CirrusMD). This fits well with ambulatory care that can be provided remotely and/or digitally. 3/4
