What will be the impact of the Trump cuts to Medicaid and other health programs? I spent weeks trying to find out, reporting in the field. One estimate is that the Trump health cuts will kill 51,000 Americans annually--fragile people like a widow and cancer survivor whom I met after she hobbled with her walker for two hours through darkened streets in hopes of getting care. Trump will celebrate the capture of Maduro as addressing drug addiction. But his health care cuts are estimated to lead to 101,000 fewer Americans getting treatment for drug abuse each year. Black men in Mississippi already have a shorter life span than Venezuelans -- and now American health outcomes seem likely to slide further. As of 31 December 2025, MSF is still awaiting renewal of its registration to operate in Gaza and the West Bank under new Israeli requirements, including staff lists. The Palestinian health system is decimated, essential infrastructure is destroyed, and people struggle to meet basic needs. People need more services, not less. If MSF and other INGOs lose access, hundreds of thousands of Palestinians would be cut off from essential care. We currently support one in five hospital beds and the delivery of one in three births in Gaza. In 2025 alone, we provided nearly 800,000 outpatient consultations, treated over 100,000 trauma cases, performed 22,700 surgeries, assisted more than 10,000 births, and distributed nearly 700 million litres of water.

If you are an American, you paid for those bullets, and all the other bullets and bombs used to kill nurses, doctors, journalists and aid workers in Gaza. Your own personal finances might be rough, the cost of living might be going up, government services might be becoming degraded, austerity might be imposed but there is always more money to kill in Gaza.

Starvation, once in motion, even after food & medical care become availabe, remains irreversible for so many of its victims. It's a greater sin than the sin itself to be have spoken only after the sight of skin-on-bones killed Palestinians. An unforgiveable "theology of empire."
