@InvictaOrtho Important to remember the supra-patellar approach is intended to be a semi-extended approach - you have to have some knee flexion for an optimal starting point.

What’s wrong with this fluoro? 4 things at least: to make a good starting point judgment you need perfect lateral (red) and you don’t have that; starting point needs to be proximal/closer to cartilage (blue); sleeve not down all the way (yellow); and trajectory too steep (green).

60M, recently dx Lewy Body Dementia, Parkinson’s sx currently. No cognitive decline. Came to me 3 months after initial injury that had partially healed then new injury refractured. Now 6 months post op ORIF with hypertrophic nonunion. Infection workup negative.

@Ortho_Rudolph Looks good and if I recall correctly 2 tightropes do provide some axial stability based on biomechanical studies. I think you did well here.

Stressed after fixing medial mall, def medial widening. Elected to go with 2 hole plate and divergent tightropes. I know this isn't axially stable, but I felt the problem was more rotational. Thoughts?
