What a week! 46 LVNC, S/p ICD had DFT test, LV just failed, Ef 20–>5, PAPi>1, SVO2 30, CI 1–emergency Tandem (CP 3.5 not enough flow and if RV fails could easily convert to LAVA ECMO—see his heart recover just on waveforms, Kern would be proud! #Cardiotwitter #TandemHeart

Deemed prohibitive risk by 3 surgeons from an AHA Mitral Valve Repair Reference Center (Michigan Medicine). Post intervention mean gradient 3mmHg, MVA 2.9cm2 by 3D planimetry. No concern for stenosis here.

The mitral valve has DIASTOLIC function as well; to FILL the LV with unimpeded blood! Images reveal significant MS as a result. If you couldn’t measure a significant gradient post clips, something is wrong with data @ovidiogarciav @BoYangMD
