Among patients with atrial fibrillation at high risk for stroke and bleeding, left atrial appendage closure was not noninferior to medical therapy in reducing the risk of stroke, embolism, major bleeding, or death at 3 years. Very cool technique to clean out segmental clot

A unique use of HDR! High-risk saddle PE (now called stage D), in a patient with baseline cardiac index 1.2. Cleared out all right lung except the lower branch (full of clots), attempts to wire or manipulate the penumbra catheter in the branch failed. So I borrowed the coronary #CTO #PCI technique, engaged the cap with the Select catheter. Some contrast to stain, then "wired" the branch with the catheter over which I used the @PenVascular catheter to clean the branch! I think coronary CTO operators can have a solution for any vascular bed! #CardioX

Case of the Day - 179 - CT Lymphangiography for Lymphatic Duct Disruption and Bilateral Chylothorax 63-years old, CKD on dialysis for 4 years developed bilateral chylothoraces 3 months ago and was referred for a CT lymphangiogram (CTL) While there are different ways of doing lymphangiograms, a relatively new technique is CTL with intranodal injection of non-ionic iodinated contrast and rapid scanning within 2-5 minutes. The video explains what I did - injecting a retroperitoneal node in the prone position instead of an inguinal node with a successful CTL showing lymphatic duct obstruction at the D5-6 level. The paradigm has changed. This procedure is now simple and can be performed by anyone who can put a needle into a node proximal to the site of obstruction with intranodal injectino of regular non-ionic iodinated contrast. @farkomd I remember being shoved/fascinated when I learned about how much the SFA bends in half when we walk - explains so much why stents occlude. Either the metal is bent too much or the artery kinks of distal to the metal. I was similarly fascinated to read this about the tibials.
