@theABofPharmaC @mlmcpherson Right. In the hyper-acute setting, such as the ED, a patient with Bup already on board can be a bit trickier to manage. Discontinuing it fractionating isn't a option.

-Continue Bup but change to q6-8 hr dosing & add short acting opioids
Or
D/C Bup and treat with short acting opioids with high affinity to mu receptor (fentanyl)

ER1 0700-1530
ER2 1200-2230
ER3 2130-0800
Recently created a swing shift 1100-1930 to add additional coverage👍🏼
