It is perhaps ironic that a tweet meant to belittle Ayurvedic psychiatry ended up being a masterclass—delivered by a handwritten prescription. When @theliverdoc asked other doctors to weigh in on the pharmacology behind a prescription issued by a psychiatrist at NIMHANS, he may not have expected the answer to come from a software engineer. I had to learn pharmacology to write very sophisticated drug to drug and drug to allergy interaction rule engine. So, I am not just a regular software engineer. So here we are. This is a public service announcement for his patients and for the medical community. First, some context. NIMHANS (National Institute of Mental Health and Neurosciences) is arguably Asia's finest institution for psychiatric and neurological care. It is not a place where seats are bought through capitation; its prestige comes from merit and academic rigor. The prescription in question wasn’t scribbled hastily—it was beautifully handwritten, conveying the kind of thoughtfulness and care that technology can’t replicate. That handwriting alone is enough to reassure many of us that the mind behind it was attentive, deliberate, and deeply committed. Now, to the pharmacology. Kalyanaka Kashaya: A Masterclass in Polyherbal Neuropharmacology Let’s start with Kalyanaka Kashaya, one of the most sophisticated polyherbal formulations from classical Ayurveda. Mucuna pruriens, a primary ingredient, contains L-DOPA, the direct precursor to dopamine. If that rings a bell, it's because Levodopa, used in Parkinson’s disease today, is a pharmaceutical derivative of the same compound! While it’s true that unmodulated dopamine can exacerbate mania, Kalyanaka Kashaya doesn’t act in isolation. It also contains Valeriana and Jatamansi, which act on GABA_A receptors, making them pharmacological cousins to benzodiazepines, but without the same risk of dependence!! Triphala and other antioxidants in the formulation help counter oxidative stress, a condition strongly associated with mood dysregulation and bipolar episodes. So, yes: doctors who use Levodopa should know that its original source was a legume revered in Ayurvedic psychiatry for centuries. @theliverdoc can sit down now. Medhya Rasayana: Where Nootropics Meet Neuroendocrinology The second formulation prescribed includes three powerhouse herbs: Ashwagandha (Withania somnifera): This is a GABA-mimetic, known to reduce cortisol and stabilize the HPA axis. Multiple randomized controlled trials (RCTs) have confirmed its efficacy as an anxiolytic, antidepressant, and anti-agitation agent (Chandrasekhar et al., 2012). Shankhpushpi (Convolvulus pluricaulis): This mild tranquilizer also enhances cognition by inhibiting acetylcholinesterase. It's particularly helpful in bipolar patients with executive dysfunction. It also has anti-seizure properties. Jatamansi (Nardostachys jatamansi): Already discussed above, Jatamansi works on the GABAergic system and is sedative, neuroprotective, and anxiolytic. Ayurvedic psychiatry doesn’t suppress symptoms—it balances systems. When @theliverdoc dismisses these therapies, he’s not just being disrespectful—he’s overlooking a system of care that has been studied, validated, and now, increasingly respected by pharmacologists around the world. Gut–Brain Axis: Avipattikara Churna The third medicine prescribed works on the gut-brain axis, an area modern neuroscience is just beginning to understand. Bipolar disorder is linked to gut dysbiosis, particularly during manic episodes. Herbs like Triphala, ginger, clove, and Haritaki reduce systemic inflammation and promote microbiome diversity. Triphala in particular has been shown to lower serum cortisol and reduce inflammatory markers, aiding in both psychological and physiological balance. Far from being random herbal mixes, these are targeted, layered interventions. They don’t just mimic Western pharmaceuticals; in many ways, they predate them. A request to @theliverdoc .
