@nigel_wylie So, because the NHS has chosen to provide primary care in this way, all employees of primary care should be at a disadvantage compared to other NHS colleagues? The system needs to be fair to all, it can't pick and choose when general practice is inside or outside the NHS.

@RammyaMathew You must have known that GPs are independent contractors when you chose the profession?

I've been a GP ever since I qualified. I thought I worked for the NHS but am finding out, caught in the middle of a ICB restructure - that I'm not classed as an NHS employee.

@doctor_katie @TheBMA Yes, I'm a BMA member and am raising it through our responsible officers. Have contacted Amy also. Thanks for your help Katie.

@RammyaMathew @TheBMA This depends on your ICB employment contract. Are you a @theBMA member? This is ripe for discrimination challenge. The BMA need test cases to support - where GPs have chosen/been forced to remain contracted on 0 hours & payment in lieu on return of invoice it’s much harder

@JABJEB99 @TheBMA @doctor_katie Awful, sorry to hear this. I think people only realise these idiosyncrasies when unfortunate events arise and then dont have it in them to try and put things right, it needs to be system led.

@RammyaMathew @TheBMA @doctor_katie was told continuous service for GPs “hadn’t been tested in court”. In a truly awful time - this made things so much worse. I wish you all the luck in sorting this out.

@clairevhooper @TheBMA @doctor_katie And worse still, GPs in clinical leadership roles in ICBs are being moved on to AfC as there is no other 'appropriate contract or payscale'. We are walking head on into devaluing the extensive skills and experience GP clinical leaders bring. This needs addressing urgently.

@RammyaMathew @TheBMA @doctor_katie I am in the same boat. 24 years in NHS as GP but only 8 years recognised for redundancy with ICB. They only recognise employees under agenda for change rules but we aren't AfC as doctors.

@doctor_katie @TheBMA Thanks @doctor_katie - I'll get in touch w/ Amy and Kim. Our time as salaried GPs isnt recognised as NHS service in the context of redundancy/ early termination of our ICB contracts. Consultants on the other hand will have their full NHS service recognised as employees of trusts.

@RammyaMathew @TheBMA You should have protections - which are you referring to? The model salaried contract is based on the consultant contract Sessional GPs are represented by Sessional GPC Co-chairs Amy Small & Kim Rollinson 👍

Private longevity clinics typically offer access to a wide range of tests and biomarkers. @RammyaMathew writes about how this is often an environment where there's a temptation to conflate innovation with benefit bmj.com/content/392/bm…

For doctors working in private longevity medicine clinics, longer consultations and a focus on prevention makes a refreshing change from the relentless firefighting of 10 minute NHS appointments. Yet it also raises ethical dilemmas, says @RammyaMathew bmj.com/content/392/bm…

@ch323 @bmj_latest Indeed, after writing I did think of my parents generation who 'dress up' to see the doctor. Is it because they prefer a more formal encounter? Or is it because it allows them to feel more in control?

PSA screening media coverage is currently very poor. Various charities are not helping but instead adding to the confusion and hype. The Times today carries another high profile individual story of how they were "absolutely" saved by screening with PSA testing.

Wow, I started out as a medical student @UCLMS twenty one years ago. I remember the hospital being built and opening. I have fond memories of it as a medical student - so many excellent teachers. What a remarkable institution it has become.
