biggest change is increasing support for early CRRT to remove NH3 (to reduce elevated intracranial pressure)
now rec for clinically obvert encephalopathy (Grade 2-4)
if admitting to ICU for ALF & encephalopathy should just dialyze

Finally, graduated from #criticalcare fellowship. It has been a strange but fulfilling journey, going from an associate professor back to being a trainee and now returning as #NephCrit with a broader skillset and outlook. Grateful to @MontefioreCCM for the training and to the leadership at @MCW_Nephrology @RegnerKR_Neph for encouraging and enabling this sabbatical. I learned a lot along the way, deepened my #POCUS experience and built #TEE skills. Above all, none of this would have been possible without the unwavering support and the immense emotional sacrifice of my wife.

1/n pt w/CKD (unknown etiology) arrives to clinic for follow up. Baseline Cr 1.7-2.1, past 🚬, base UPCR 0.4. Two months prior to the visit, Cr had ⬆️ to 4.2 and PCP stopped ARB. New labs: Cr down to 2.5. ARB was resumed at 50% reduced dose. 2 mo later, new visit: Cr back ⬆️ 3.8

In severe AH AKI, ATN is the most common AKI phenotype.
➡️ATN 44%

➡️PRA 30%
More work is needed to understand the mechanisms of AKI in AH.
