@Derek_Leong_RPh @FralickMike @dunbar37 Sounds simplistic, but frankly I ask the patient what they are comfortable with. Most never want be admitted to hospital again, so it’s an easy decision. Then I make sure not to make them feel they are missing out by not restarting it.

@Rene_Wong @FralickMike @dunbar37 Richard and I were discussing earlier about whether these patients are committed to insulin like typical DKA patients, or whether stopping the drug is sufficient and they can still have OHAs for their DM2 management. What is usually done for these patients?

Saw a patient with @dunbar37 today who has euglycemic DKA in setting of SGLT2 inhibitor. Important for prescribers to discuss sick-day management (SADMANS) with patients, and to realize this class of drug isn't for everybody despite guideline recommendations/EMPA-REG

@Derek_Leong_RPh @FralickMike @dunbar37 Sadly many pts feel they MUST stay on them after told by MD they are “best” or evidence based drugs, regardless of how many UTis they have had or other AE’s. Need to go beyond sick day discussion, and give pts “permission” to not take the med at all it it doesn’t fit

@FralickMike @dunbar37 Thanks Mike! Though credit goes to our ER/ICU colleagues who managed patient overnight and figured out those details. We took over in AM when gap closed and IV insulin stopped.

@Derek_Leong_RPh @JesskelleherLiu Has been effectively dropped from the diabetes “status quo”. Has a potential role IMO for the Pt requiring +++ insulin but now with other options I can’t recall last time I prescribed

Are glitazones still being used frequently for DM2 management? Haven't seen one in a long time until @JesskelleherLiu and I saw a patient today who came in on pioglitazone - we stopped it since they are known to have HFrEF and admitted for NSTEMI

@Rene_Wong Our students were invited to bring hockey sticks for the remembrance wall at each campus. Individuals or groups and classes decorated and signed sticks. Some students did wear a jersey to school today, but our school initiative was to place a stick out of respect and remembrance.
