#CC #PT582 This SR shows little evidence of US being better than a placebo. Due to this, we shouldn't be overusing US. The placebo effect is good, but it's only short term. As DPTs, we need to strive to make lifelong changes. That starts from the inside, not from a wand.

#PT582 #AJ From my research, when its NMES vs exercise, then exercise should win. But there is nothing wrong with ALSO utilizing modalities if we see results. Exercise does exactly what all surface-level modalities do, so we need to educate our pts on that.

#PT582 #RI Last Thursday was an absolute stellar discussion. Glad to have been a part of such well-thought ideas and evidence.

@SaraKni80449195 Yes, as we know, the SAID principle is essential! Awesome point Sara. It seems that a soccer player would not become better at soccer by having a machine contract their muscle for them.

#PT582 #RI Russian E-Stim can improve muscular strength, but this treatment can induce temporary pain. I don't think it would serve athletes well to only utilize this, since they would lose out on sport specific training, which is known to reduce injury and increase performance.

#PT582 #RI Using E-Stem for competitions would be considered another modality to enhance recovery and performance. I would put it under the same modalities such as cryotherapy and BFR.

@AnnaReiserSPT Definitely! That is such a good idea. It kind of relates to what we discussed in neuro today!

@Riley_shae98 Strengthening is important but so is the neuroplasticity component. I think you're on to something with watching the contraction, maybe this could be incorporated into some visualization techniques to improve athletic performance! #PT582 #RI

@RobertGorman18 Exactly. Most modalities are surface level as well. The body has the power to do healing so it is important that we utilize that!

@Riley_shae98 As far as ethical issues regarding Russian e-stim, there don’t seem to be any as this is largely a therapeutic modality. Inhibiting the use of this could be detrimental to those athletes attempting to recover from injury. #PT582 #RI

@AnnaReiserSPT Anna, so true. Do you think strengthening muscles alone has a benefit in a sport? Does watching our muscles contract have a neural/brain benefit?

@Riley_shae98 #PT582 #RI interesting topic Riley! I don't see any concerns with it because using estim to strengthen muscles is commonly used among athletes and does not chemically alter their muscles.

@Riley_shae98 I think an ethical dilemma would possibly present if athletes were attempting to use e-stim during competition, although I am not sure if that is even feasible. Regardless, PTs must demonstrate responsibility and foster the Pt's understanding and respect of modalities. #PT582 #RI

@kdavidnau Yes! I have had many patients that have used it as a tool to relearn how to contract their muscle as well. Not just for strength gains!

@bonniediamond53 Bonnie, totally! Do you think since it just targets a single muscle, that it won't really affect the neural skill component?

@kolbyareed Kolby, this is a fantastic article. Do you think that considering general rehab tools as "cheating" ever right? Is it important that we use all of the tools in our power to get our patients healed and back on the field, court, etc? #PT582 #RI

#PT582 #RI WADA bans drugs for having 2/3 criteria. Harms the body, enhances performance, against the spirit of the sport. Until its proven to harm the body when used properly or show a great benefit to performance, I say athletes should use it. ncbi.nlm.nih.gov/pmc/articles/P…

#PT582 #RI I don’t think it would be considered cheating because it’s just another form of training muscles. it’s fairly easy to gain access to the e-stim machine so there’s no unfairness regarding availability. It’s a method of conditioning the body to perform at its peak

... much unless they are putting in the same functional, sport specific training as everyone else. In my view, these types of modalities that could improve performance in high level sport performance can be monitored for safety without being outlawed

#PT582 #RI I personally think if its being used responsibly/safely, it is within ethical limits for athletes to use. Access to tools like Russian EStim and BFR likely differ, giving some athletes/teams an "edge," but I also don't think that added strength from Russian means...

I do not think that using e-stim should be considered cheating. It is something that can be used in conjunction with training and is available to everyone so it does not create an unfair advantage. @PT582 #RI

#PT582 #RI I do not think there are any ethical complications during training, but if they managed to use the technology during their competition it could provide an unfair advantage. I think cheating would be if it taught us to go beyond what our muscle could do anatomically.
