@LucyGoBag No real excuse for this as EPRR in Health has been teaching and advocating the Loggist role and use of log books for decision makers for a long time, in addition Clinical notes should be to a standard that they can/should be used.

This is something regularly flagged in multi agency major incidents such as Grenfell. Health and particularly clinicians not always trained to log or record in same way as other agencies. Some improvement in pandemic but still on our list of 🚩 concerns

Contemporaneous notes: Apart from one person. No health professionals at Inquiry had their contemporaneous notes available. (Hence so many - “I can’t remember”) Can health professionals attending the Inquiry remind themselves there are strict rules & regs about retention of hand written contemporaneous notes, they form part of the patient record….what a shambles. @JoelMoore98 @BBCNottingham @CareQualityComm
