I’ve seen someone else on here suggest that the retired doctor who wrote this was a “junior” doctor in 1974 when he was “happy with £7,500” Just so we’re all clear on the maths: £7,500 in 1974 in today’s money is: 🔹 £114,756 by RPI inflation. 🔸 £72,046 by CPI inflation.

🚨 CHIP-BCIS3 randomised 300 patients with LVEF ≤35% undergoing high risk PCI to either micro axial flow pump or standard care ➡️ mAFP did not reduce major adverse clinical outcomes ⚠️ There was an excess of CV death in patients who received PCI with mAFP #ACC26 @BCIS_uk

The SCAPIS study published in JAMA ~25,000 🇸🇪 participants, ~8 years follow-up. Adding coronary CT angiography #CCTA plaque information improved event prediction beyond clinical risk and calcium score. This is the first study showing the added value of CCTA on top of the calcium score for risk prediction. 🎯 What to look for: lipidic soft (non-calcified) plaque. 👀 Practical takeaway: Consider a CT angiography for risk stratification on top of the calcium score. Intravascular imaging in the UK- as part of my live #BCISACI national data talk ➡️34% of all PCIs are undertaken with image guidance in UK with wide variation amongst centres. LMS PCI close to 80% ➡️Even ACS cases close to 30% of cases are imaged. I would argue that we are still not undertaking imaging enough- with little data on whether we are actioning the findings. ➡️imaging doesnt improve outcomes without using information it provides.

Unique strategy: patients randomized before lesion preparation — unlike prior DCB studies. ✅ Non-inferior for 1-year TVF (5.3% vs 4.4%) 💀 Death, MI, thrombosis all similar 🧠 80% of DCB patients avoided a stent ⚠️ Slightly higher TVR (3.3% vs 2.1%) 🔍 Low intravascular imaging use (~16%) outcomes could be even stronger with optimized prep & imaging guidance.
