$sls short Conclusion: high probability of failure I’ve kept it short because sometimes it really is just that straightforward. $sls in a memesque frenzy, has traded very irrationally due to its short interest most of this year. Folk piling in trying to cause a short squeeze not really understanding the mechanism or the PoS. Readout should be in a few days/ weeks and is primed for failure so even at $12, plenty of meat on the short. Conscious that people reading like the smoking gun up top, so long story short, it’s the design. Standard error on the log hazard ratio is roughly 2 over the square root of events. At the 80 death final the critical HR is about 0.64. They powered to 0.635. The trial was built to land exactly on the sig line with zero margin for error/ cushion. So if the control arm has any sort of outperformance the trial will miss. And the control WILL outperform… I mean if you look at pts they enrolled.. pts who relapsed, got salvaged, responded again, and recovered enough to enroll. That is a responder enriched group by construction. Comparable responder and treatment naive cohorts live past 20 mo not the 8 the trial assumes. The benchmark looks too low. Then you have the mechanical trap. Required GPS median scales linearly with control. At 8 mo control GPS needs about 12.5. At 10 it needs 15.6. At 11 it needs 17.2. A WT1 peptide vaccine has NEVER shown numbers like that in a randomized trial. So I don’t see why that is going to change. Every extra month of control survival pushes the bar out of reach. And I know everyone knows this, but pertinent to add this mechanism has constantly failed. GPS was dropped across mesothelioma, ovarian, NSCLC, myeloma, MDS, and AML CR1. That’s a pretty big list. The only randomized GPS trial ever run missed every endpoint, OS p value 0.54. The closest WT1 comparator failed a randomized Ph2 in an easier setting. Peptide vaccine monotherapy has essentially never won a Ph3 on survival. Then on top of that.. the delay is not the bull signal it is sold as. We’ve seen it before in other trials like $arvn for example, it’s not a clear signal just like recruiting staff prior to the topline. All noise. Alpha was spent at the 60 event interim against a boundary near 0.51 and nothing crossed. That rules out a knockout effect. Slow accrual since is equally consistent with the control arm living long, which is the outcome that sinks the trial. I will say this though interim boundaries are conservative, so a genuine winning effect could still clear at 80. We don’t know what the BAT composition is. Having said that there remains a high probability of failure and it won’t be long till it all comes crumbling down for $sls imho.
