If you’ve had enough, get real.. You can’t all terminate your Medicare agreement. Too late for that. You’d go out of business. If you could get your collective acts together you could close to all NEW Medicare patients. No significant impact on revenue. But big challenge for Medicare. Only then, will someone offer to listen to your demands.

I would like to thank Washington for finally finding the waste in American healthcare! It was a physician. I know. I was as surprised as you are. Specifically, a physician who evaluated a patient and treated her in the same visit. One trip. One parking fee. The efficiency of it makes me ill. So CMS are going to pay him half for the second thing he did. As they should. Now, I was not worried for a moment. I read which rule it was in. The cut lives in the rule that pays the physician. My facility fee lives in the other rule. My drug spread lives in the other rule. I collect three fees on one patient in one hour, and no one has ever once called me a duplicate. If duplication were a sin, I would be the cathedral. But the rule walked right past my tower and knocked on a dermatologist with a table and a light. Funny, how it knew the address. And they did it in the name of making America healthy again. There is a television doctor running Medicare and Medicaid now. I could not have written it better. I did not do this. I merely benefit from it. There is a difference, and the difference is beautiful. -Rojas out.

The most valuable asset in American healthcare is not healthcare, it is a halo. Ascension calls itself a Catholic ministry. The IRS calls it tax-exempt. Schedule J says its CEO received $14,086,474 for the fiscal year ending June 30, 2025. It is in the filing. And the question isn’t whether executives deserve to be paid. The question is why taxpayers subsidize organizations that eliminate independent physician competition while claiming charitable status. The receipts are public.

You write a comment in the lab order explaining why the patient needs daily labs and that you insist the order be repeated daily. Then you write in your note why daily labs are indicated. Then you copy both to risk management and state you’re not responsible if there’s a complication for a missed lab.

But they are trying to save money at the expense of increasing “provider” … busy work They figure if it’s too much hassle we won’t order daily labs And if we really need it we have pay a “toll” - which is to remember to order the labs Every Single Day I have leukemia patients who unequivocally will need daily labs. I’m a very good saver and have been since age 18. Nobody on his deathbed, with any sensibilities, will wish he’d made more money at the expense of time with his children. I have friends who work low wage jobs but spent every waking minute with children hunting and fishing and everything else. They are truly wealthy.

Most people don't save anything, so obviously this isn't the norm. But I've seen a lot of people run into "compounding brain rot." Where they won't take that trip because, "I am missing out on ____ returns." Too far on the other extreme for sure.

@SpaceCubElroy @DutchRojas ‘Well I can’t see the images if you go somewhere else’-is a pretty common excuse relayed from my patients. Or they claim quality issues at other systems. Never actually refuse, but patients are reluctant to push for obvious reasons.

@X0_1_7ex @DutchRojas One of the requirements of avoiding Stark violations is written agreements with physicians acknowledging that the patients have a right to utilize any provider. It’s intentionally confusing and I don’t fault you for thinking you may be correct, but you aren’t.

Maybe AI is a blessing in disguise. How many professionals will be able to work for themselves rather than join an exploitative firm. Joining firms used to be a necessity due to capital cost and human resources costs. But a recent grad and a computer with AI assistance can do as much now. The infrastructure is no longer necessary. $1200 is bad enough, but in a hospital it would’ve taken two to three hours and the interpretation would’ve taken an additional day or two. Dr’s office is 15 minutes at most. The hospital would most likely generate two bills. One would, more than likely, come from a pulmonologist for an additional fee.

My friend Luke texted me the estimate his hospital handed him for a spirometry test. You breathe into a tube and a machine reads your lungs. His good faith estimate: $1,200. I sent him to a direct primary care doctor here in town and he paid $50. Ascension files its taxes as a nonprofit.

The copay is set by your insurance company. It’s a component of the participation agreement with Medicare, United Health Care, Blue Cross etc. The physician’s office has zero say in the matter. It’s a great idea, but the insurance companies are who you need to discuss the idea with.
