How will it all work in the near-term future as we integrate screening+treating folks for memory loss and use of digital technologies? Will it be critical for Alzheimer's and Parkinson's? You bet it will. Enter the Digitized Memory Clinic by Gramkow and colleagues as laid out in their new paper in @Nature Reviews Neurology. Key Points: - The authors suggest a simple tiered approach. - Tiers 1-3 are diagnostic and monitoring and include critical person-centered assessments. - Tier 4 is post-diagnostic care. My take: I love that the construct does not make the physician the sun. The person w/ memory loss is the sun, and all healthcare team members, not just the doc, orbit around them, and of course use the digital technology. I love that this will bring care to more people and do it in their homes. The AI-driven software is squishy today, but I can see this developing quickly, and of course will feed much better clinical decision support tools. This will facilitate the critical need for disease tracking, All of this will be much better once we break through with more effective treatments for memory dysfunction. This idea will drive clinical trials faster and this will be a very good thing. Is hearing loss associated with an increased risk of new diagnosis of Parkinson's disease? Yep. Neilson and colleagues @JAMANeuro show us association in a dose-dependent manner. The risk was interestingly attenuated by hearing aids. cutt.ly/8eDE8762 #parkinson

Do you know the differences between MoCA vs. mini-mental status examinations vs.multi-hour neuropsychological testing? Drs Fred Cohen, Shiv Sudhakar and I sat down @FoxNews as a chance to educate the public. Folks remember the Goldwater rule as this is about education and not speculation. Did you know? "There is rock-solid data that 5% to 8% of folks over the age of 60 will later develop dementia. This data was partly why Congress passed a law prohibiting commercial pilots from flying after the age of 65." "Most cognitive tests don’t consider the mood of the individual – so if someone is depressed, that person may score lower." "The MoCA and MMSE are considered by experts to be broad and shallow tests, and therefore should only be applied for screening and triage." "Folks should be aware that the MoCA and the MMSE are not a replacement for neuropsychological testing, even if a person achieves perfect or near-perfect scores." Do you know the ‘Pants-Sign’ as a predictor for falling in Parkinson’s? Jansen and colleagues add a nice tip both for clinicians and folks w/ #Parkinsons @journal_PD Key points: - A history of falls is the most 'established predictor' of future falls in PD. - Difficulties 'putting on pants' while standing' was considered by the authors as a predictor of falls. - The author's defined their sign: A ‘Pants-sign’ is present when 'people who resort to putting on their pants only while seated.' They studied 264 folks. - The Pants-sign was uni-variably associated with a future fall but not an independent predictor in the multivariate logistic regression. - Predictors: higher MDS-UPDRS part III scores and history of falls. - Important point: Those without falls in the previous year (n = 189), the Pants-sign was NOT associated with future falls. My take: It is simple to ask this question about how you put your pants on when taking a history from a person with Parkinson's. I personally like the Pants-sign, however it clearly has limitations. Previous falls predict future falls, and one clear message from this and other data is that when falling starts, your management and safety plans should immediately shift. Perhaps, we should more frequently offer sage advice for the person w/ Parkinson's: If you are falling, consider the possible benefits of sitting when putting your pants on. content.iospress.com/download/journ… @basbloem @NonnekesJorik
