Because it’s expensive and someone(s) convinced themselves they were good to make it another run, but they didn’t account for juicing the units post Iran to try to get as much rate as possible, and that hotspot they were monitoring wasn’t read right and maybe not even the worse, and those thickness measurements they took, weren’t in the thinnest area, and those tube life calcs they made didn’t account for all this times they ran it hotter than their thermocouples picked up, or maybe they didn’t have any, or maybe their IR tech had the wrong camera settings, so they just fly blind until- well this happened. Basically almost every refinery out there these days
