Discussion (cont): The key molecular hallmark of secretory carcinoma is the presence of a recurrent chromosome rearrangement, generating ETV6-NTRK3 Fusion. Immunohistochemistry for pan-Trk is positive in this case, a sensitive and specific surrogate for NTRK rearrangement.

Discussion (cont): Intracytoplasmic vacuoles and eosinophilic secretory material in luminal spaces are characteristic for this subtype. The tumor cells are negative for ER, PR and HER2.
