🧵1/ Another #HRS2026 #JACC data on Purkinje De-Networking (PDN) for recurrent VT/VF! MMVT patients + prior polymorphic VT/VF, adding PDN during VT ablation improves outcomes. Results at 12 months: VT/VF-free survival: 🔰81% PDN+VT vs 49% VT ablation alone 🔰Absolute difference: 31.6% 🔰HR for recurrence: 0.29 (p=0.02)

@EladAnter I like your mechanistic hypothesis. I was thinking that the Purkinje network may promote salvos of PMVT that may initiate MMVT. I’ve seen this occur spontaneously and with PES.

@True_EP Really nice study and original thinking. What do you think is the mechanism? DADs from Purkinje fibers creating premature beat(s), leading to unidirectional block and VT initiation? Would be interesting to test with PES from Purkinje sites in a few patients, especially those noninducible from other locations.
