#2 *⃣CIED reset and damage with Sphere-9 monopolar lattice-tip PFA (Nair et al.) @Drdevignair @VivekReddyMD ➡️ Case #1 ✴️SVC PFA in a pt with CRT-D resulted in abrupt termination of energy delivery, temperature rise at the catheter, and irreversible loss of BiV pacing. ✴️CRT-D generator was non-interrogable & required replacement despite preserved lead integrity, strongly suggesting direct device-level injury; ➡️Case #2 - PFA PVI/CTI resulted in complete software reset of an Aveir AR leadless pacemaker, which subsequently recovered. 🧑‍🎓Monopolar energy delivery—where current travels between the catheter and surface electrodes—may facilitate energy coupling into adjacent conductive structures, resulting in either transient software effects or irreversible hardware disruption. heartrhythmjournal.com/article/S1547-…
