@E_ke_ne Every time knowledge . What stupid knowledge gap. What aproko said is what it should be, however in our teaching hospitals and medical centres, how many times do doctors run the so called Endoscopic test before prescribing Omeprazole, antacids , even ULSAKIT etc?

Look at the context of the discussion, he is talking to someone who is a non medic and he said ulcer in the layman's context. When a patient eay they have ulcer its a non specific terminology that encompasses multiple different diagnoses that present similarly, dyspepsia, PUD e.t.c. all of this diagnosis have similar management PPIs, Antacids e.t.c. while you need endoscopy to confirm a diagnoses of PUD, you do not need to confirm the diagnoses before you start treating because the initial diagnoses of undifferentiated dyspepsia(what laymany calls ulcer) is clinical. Endoscopy is very invasive and it is not the recommended first line of diagnosis. There are specific indications for endoscopy. So i repeat again that you dont need endoscopy to diagnose ulcer (dyspepsia) or commence empirical treatment. Otherwise you want us to do upper GI endoscopy for every patient that comes to the hospital to complain of epigastric pain.

The fact that you don't know how many times they do endoscopies in our teaching hospitals is exactly why you should sit out of clinical diagnosis conversations and dispense your tablets. You are drowning. It is an unfamiliar sea. You came to a tweet pointing out a knowledge gap to prove his point.

Again, I have seen that too many people have already explained this to you, so I won't bother repeating their error. The fundamental problem is that you are in the terrain of clinical diagnosis and acting with contempt and authority. This is not your field. There is no Doctor arguing drug formulation and dispensing with you. You are not trained in clinical judgement and diagnosis. That is the sole reason none of the clarifications have been able to get through. Simply sit this one out. Maybe return to it and read their replies again when you are humbler, you may actually learn something you will carry on.

@dr_igwe_ @solobliss1 😁😁😁. In our hospitals here in Nigeria ? Ask Nigerians that were told by medical doctors that they have ulcer ,how many of them did endoscopy before diagnosing their ulcer. Is it not Nigeria hospitals? Our teaching hospitals and medical centres? Or you want to talk about GH?

@Ucg4sure @solobliss1 The treatment is for dyspeptic symptoms and H pylori eradication and not for ulcer. Without endoscopy, no certainty that there's ulcer, it could be gastritis or even a gastric cancer

@Ucg4sure @Stoveiscoming I’d ignore your uncouth comment. Let’s get to the point. Read the last sentence I wrote !!!! Omeprazole isn’t used for Just Ulcer . It’s used for gastritis, H pylori eradication, prophylaxis with NSAIDs. ULCER is an endoscopic diagnosis. Learn to be civil in discourse
