The “12 months” column on the right is referring to those intervals, not how long participants had been receiving care. That’s explained in the paragraph and figure above this table.

The former is essentially plastic surgery, which minors can already get with parental consent. It’s also usually after HRT administered by a doctor. Bottom surgery is always 18+ in the US.

Had the steps been taken that needed to be taken, then they would have been able to get the fire under control and stop it.
