Reminder - I’m 39 years old, not on lipid lowering medications and baseline scan showed mild plaque in LAD and CAC score of 4. Strong family history of early CVD (dad had heart attack at 41, grandpa in his late 40’s). Fortunately low Lp(a) and my ApoB has been under 80 mg/dl for the better part of a decade thanks to my diet. Some other recent lab markers: HbA1c: 5.1% Fasting glucose: 82 mg/dL (down from 87) Fasting insulin: 3.2 µIU/mL VO2 max: 53

I had my CT angiogram results from @BudoffMd sent to Heartflow to analyse - comparing a 2024 scan to a scan 16 months later. Once I have the results and review them in full with cardiac imaging experts expect an episode. What’s your prediction?

Why is there a huge drop in modern men's testosterone? There are a few big reasons - one of which hardly ever gets mentioned: Older tests were using immunoassays to measure testosterone. This is not a direct measurement. It infers the amount of testosterone in the blood. Immunoassays consistently over-estimate testosterone levels. In the late 2000s labs switched to mass spec - which directly measures the amount of testosterone in the blood. When researchers re-tested stored blood samples from men previously tested by immunoassays, the proportion falling below 300 ng/dL nearly doubled. This is why the Endocrine Society revised the lower limit from 300 ng/DL to 264 ng/dL. The threshold dropped because the test got better. Across the last 50 years, the older immunoassay values wear artificially reading high, while the newer mass spec values read more accurate.

And what population is the established reference range based on? Is it millions of healthy gym-going adults? Why were reference ranges 30% higher 50 years ago? I wish there was an aspect of public health we could point to that would influence the averages plummeting in the way they have. I'm sure you're right though. People should aspire to hormone levels of a sick, dying population, not a healthy one. Silly us. 🙄

To grow muscle effectively you must: - use a controlled, full range of motion - use proper form and focus on high quality contractions - eliminate body English and momentum based movement - continue reps until you are within 1-2 reps of failure (if you’re a beginner, just use failure as a heuristic) - track all weights and reps - constantly try to beat the training log and improve performance - rest 3 minutes minimum between sets - eat and sleep enough to support recovery If you’re missing just one of these steps, it will compromise growth. Bookmark this tweet + refer to it as your checklist.

It's an old paper (2015) - but I wouldn't be surprised if the data were similar. Maybe even worse. 25% of men jumping on TRT never even get their initial/baseline testosterone levels tested. That's crazy to me.

I don’t know who needs to hear this but… If your “TRT” has your test well north of ~1000 ng/dL, that’s not TRT. That’s PED use. TRT = restoring normal physiological levels, not pushing beyond them.

Depends on your budget. But if you're looking for something affordable that's good quality I'd recommend a Seiko 5 automatic. Case size is between 37-38mm. Price is usually around $250 (sub $500 for sure). Lots and lots of different styles to choose from too.

Go 31x34 - that 33" inseam is elusive. Same with a 31". 31x34 and then go to a tailor and get them hemmed. And if you can't find a 31" waist - do a 32x34...and get the waist taken in and inseam hemmed.

I recommend a 8-10" drop which is much more achievable. Literally ANY guy can get there. 10-12" - a little more ambitious.
