A MUST READ! This is a most important article that echoes the past for most of us who chose Family Medicine or primary care. It needs attention, discussion, and action now. Allow me to share my thoughts (hang with me): 1) The shortage of primary care physicians (now and projected) is nothing new, it is critical and needs drastic change and solutions 2) The article points out the continued 'hidden curriculum' in medical schools (mostly allopathic) in steering smart students AWAY from primary care through culture and faculty comments - this is REAL, it happens, and needs to STOP! 3) It remains idiotic that 12 allopathic medical schools continue to this day to not equally recognize Family Medicine as a major discipline and not have a full Department! 4) It is true that current payment models and future income potential greatly impact specialty choice and needs attention to benefit primary care. But I am SHOCKED to read in this article that some of my FM colleagues believe that medical schools have little influence in changing people's career choices! Just look at the success rate of Osteopathic schools (and the University of Washington) compared to most other Allopathic schools - they have much higher rates of students choosing primary care so there is a 'secret sauce' being used that has such a positive impact! Medical schools can and must do better! And our FM Departments must insist on this and NOT say 'we have little influence'. 5) How about this drastic change - lets begin to hold medical schools accountable for their % of primary care graduates based NOT on what specialty they indicate at the time of graduation but what those students are actually doing at least 5 years later. This avoids the 'Dean's Lie' that happens at every medical school each year in announcing - 'we have over 50% going into primary care' - while knowing full well that most will subspecialize by year 5 or more beyond graduation. 6) And lets consider enforcing the above by better distribution (and perhaps withholding) of GME funding if schools do not produce enough primary care physicians. OR even better, consider distributing NIH research funding based somewhat on this goal of producing primary care doctors. THAT will get the attention of medical schools and academic health centers! 7) Lets finally get serious about the admissions process and move beyond test scores and GPA - they do not define the best and the brightest. 8) We do NOT need schools specially focused on producing primary care physicians - that lets the rest off the hook! Stupid policy and effort. 9) Lets NOT have tuition free medical schools across the board. Instead, lets have fee tuition (or large scholarships) for those who actually choose FM, General Internal Medicine, or General Pediatrics AND who are practicing these disciplines at least 5 or more years after graduation.
