@Zhen_MD Thanks for that, but I don’t buy any of their reasoning. Peds residents manage those same complex patients for 3 years. “Resource utilization” and “multidisciplinary care coordination” can be refined as junior faculty, rather than as underpaid fellows for 2-3 years.

Why are hospitalist fellowships forced on peds residents, but not IM residents? Same inpatient/outpatient ratio residency structure as far as I know. IMO, it’s easier to convince women that they don’t know enough (even when they do) and scam them into this redundant training.

As a financial advisor who is allowed to own a financial advisory firm, I find it absurd that a physician can’t be an owner of a hospital. 🤷🏼‍♂️ How was this ever conceded??

From an employer’s viewpoint, APPs are cheaper and can do a “good enough” job so they’ll always advocate for that. Private equity looking to flip a healthcare organization to make a return on their investment will also be interested in finding that more affordable solution. As for the training question, resident pediatricians ask that all the time. The best answer I have heard from only one person out of all the people I’ve asked (most who can’t give a clear answer), is that hospitalist medicine in peds is no longer the routine cases, it’s a lot of complex care coordination that requires that extra training. Now… I said best answer, not an awesome answer. My counterargument would be regardless of the care, residency is still mostly inpatient medicine and if you are training at an academic center, you’re seeing those complex care cases so why the need for an extra 2 years fellowship. I guess for someone coming out of a community hospital program, it could be helpful. Community hospitals don’t require their peds hospitalist to have done a hospitalist medicine fellowship as much as academic centers will require that training.

In case you needed further evidence that #pediatrics is an undervalued speciality, I just got off a call with a recruiter from @EagleTelemed offering me $16/hr to cover a 5 bed rural #PedsICU for 24 hrs via telemedicine due to a lack of in house physicians. $400. Flat rate. For 24 hrs. No additional compensation for time spent managing pts. $400 flat rate to be immediately available at any moment day or night to manage a critically ill child by a physician double board certified in pediatrics and pediatric critical care. That’s less than minimum wage in some states. I never expect to get paid well in peds, but that is insulting. #MedTwitter
