Aerobic exercise can reverse age-related brain shrinkage. In older adults, just one year of regular aerobic activity increased hippocampal volume by 2%, effectively restoring 1–2 years of brain loss. This highlights the remarkable ability of exercise to counteract brain aging. But does intensity matter? In a recent study, mice doing HIIT experienced an impressive 85% increase in new brain cells in the hippocampus—an area critical for learning and memory. What’s even more remarkable is that HIIT didn’t just boost neurogenesis in the hippocampus—it drove a 155% increase in new brain cells within the ventricular-subventricular zone, a brain region involved in olfaction, or the sense of smell. That’s nearly double the effect seen with moderate exercise. So, what makes HIIT such a powerful stimulus for neurogenesis? It turns out that lactate—a metabolite produced during intense exercise—plays a crucial role. When researchers removed the receptor for lactate, called HCA1, mice no longer experienced the same surge in new brain cells. We now have overwhelming evidence that lactate isn’t just a byproduct of exertion—it’s a key signal that helps drive brain plasticity. DOI: 10.1113/JP287328 Human study PMID: 21282661

@AJWPharm Thank you for sharing this and congrats on the publication! We have discussed this topic multiple times during my PGY2! Anecdotally, when I was on my neuroICU rotation, we actually noticed a few patients had a better response when their dose was decreased from 10-15mg to 5mg TID.

In short: Bromocriptine is an option for central fever, but is not the be all end all of antipyretics in the NeuroICU. It is worth a trial in a persistently febrile patient, but if no response is seen after the first 3-6 doses, it is unlikely to be effective

It is generally well tolerated, though. That being said - our one patient who got up to 60 mg/day did experience pretty extreme LFT abnormalities which improved after therapy discontinuation

Patience pays off - at long last, our final manuscript on bromocriptine for central fever has been published! We describe 30 pts where bromocriptine was used for central fever. It was modestly effective overall, although some patients (esp SAH patients) saw a dramatic response

IV levothyroxine does not provide additional benefit in organ procurement of brain-dead heart donors compared to normal saline. Based off of this study, OPOs are beginning to reevaluate their protocols & eliminating levothyroxine from standard care.🧠 #NCSPharmJC @neurocritical
