Hx: self-med Prednisolone 20 mg OD ×8 wk post-COVID, Occupational ICU exposure, wt loss 5 kg/1 mo, dry cough present.
Vitals: T 39°C, BP 92/58, HR 118, RR 26, SpO₂ 91% RA.
O/E: GCS E3V4M5, Rt UL power 3/5, Rt hyperreflexia, Rt plantar extensor, fundus papilledema+, RS B/L upper zone crepts, skin showing 3 painless erythematous nodules (1–2 cm).
Labs: Hb 9.8 g/dL, TLC 3200/mm³, ANC 2500, ALC 380, Plt 110k, ESR 92 mm/hr, CRP 148 mg/L, Ferritin 2100 ng/mL, LDH 780 U/L, D-dimer 2.8 µg/mL, Na 128, Cr 1.8 mg/dL, Albumin 2.6 g/dL, AST 118, ALT 92, ALP 260, Procalcitonin 0.4, CD4 180, HIV negative, serum galactomannan 2.1 (>0.5 positive), Beta-D-glucan 286 pg/mL (>80 positive).
HRCT chest: multiple 5–15 mm random nodules, GGO halo lesions, few cavitary nodules.
CT brain: 8 ring enhancing lesions (5–14 mm) involving basal ganglia and corticomedullary junction with mild edema.
CSF: OP 28 cm H₂O, cells 110 (80% lymph), protein 168 mg/dL, glucose 38 (serum 110), ADA 12, AFB neg, TB PCR neg, HSV PCR neg, cryptococcus neg.
D1 microbiology: CSF multiplex meningitis/encephalitis PCR panel –neg, CSF GeneXpert MTB/RIF –neg, Blood multiplex sepsis PCR –neg, blood cultures negative.
On day 3 developed hemoptysis, shock, AKI (Cr 2.6).
MRI brain (D3): multiple hemorrhagic ring lesions with diffusion restriction suggesting angioinvasive etiology.
BM biopsy: hemophagocytosis, triglycerides 410 mg/dL, fibrinogen 110 mg/dL, suggesting secondary HLH.
What is the most likely unifying diagnosis, underlying pathophysiologic mechanism, key diagnostic clue(s), key red flag(s), important differentials, most important diagnostic test(s) for confirmation, immediate treatment priority (infection vs HLH and why), definitive management strategy, drug(s) of choice with dose, drugs that should be avoided with pathophysiologic justification, complications anticipated in next 72 h, and overall prognosis?
