The very 1st #AblateVT for the @SHC_TriValley community 🚩 @StanfordHealth. Dual-arrhythmia NICM: LsPeAF & >25% mono PVCs/VT refractory to Sotalol/Amio. Epi #HDGrid mapping of PVC via VATS port at time of #HybridAF. EF 20 📈 60% cMRI without AAD at 8 months, ICD avoided #EPeeps

#VF trigger = 🪡 in a haystack = malignant Purkinje potential. SQ-ICD template pace “match” (conduction system, septal RV, & moderator band) too broad an area to ablate and still 🚫 match w modicum of clinical PVCs. To lose patience is to lose the battle #EPeeps

#Epeeps @CardiologyCoder @CMSGov @HRSonline Anyone working on reimbursement for conduction system pacing for CRTP/CRTD implants? These are only reimbursing as pacemakers and ICDs with our local coverage determination. It’s disappointing to correct a QRS but get punished.
