Thiamine is not used to build the enzyme protein alpha-ketoglutarate needed to make ketones. It is used to make thiamine pyrophosphate, TPP, which is a required cofactor for the alpha-ketoglutarate dehydrogenase complex. With low thiamine, alpha-ketoglutarate dehydrogenase may still be present, but its activity can fall because it does not have enough TPP. That complex helps move carbon through the TCA cycle and generate reducing equivalents for mitochondrial energy production. Thiamine → TPP → TPP sits on E1 → TPP TPP is the “carbon-handling” cofactor for the E1 step. It does not build the enzyme alpha-ketoglutarate dehydrogenase. It lets the enzyme perform the difficult chemistry of alpha-keto acid decarboxylation. Keto does not bypass the thiamine-dependent downstream mitochondrial oxidation step. That means a person can still make ketones, measure ketones, be “in ketosis” and yet still have impaired energy extraction from that fuel if thiamine-dependent mitochondrial enzymes are not working due to thiamine insufficency or deficiency. Thiamine insufficiency and even deficiency is common. Its treated based on symptoms more than testing. I put people on good quality supplementation as supports to their ketogenic diet for healing in my online program for this reason. So in closing, no low thiamine will not impair your ketone levels the way carnitine insufficiency will. But it will impair the energy production you were hoping to experience in your ketogenic diet. And as we all know from dear @MitoPsychoBio with his latest substack this morning, energy matters.

My homemade lab… Regular Pharmacies don’t have 18 mg capsules. That needs to change. Because- Mixing it in water left lots of residue and gave me inconsistent doses. Inconsistent doses can worsen withdrawal symptoms or bring about relapses. A compound pharmacy was expensive and can take awhile to make. I tried that. Unfortunately sometimes I can’t eat due to drug induced Parkinsonism. Time is of essence. This is a much better solution for me. Most psychiatrists don’t understand how to safely taper off drugs. Thanks for all your help @GeorgiaEdeMD

Absolutely sick to death of hearing about how the new @louistheroux documentary has made people think about attitudes to women. Lots of MPs jumping on the bandwagon. I mean, no disrespect to Louis - I’m grateful for the highlight - but for God’s sake… Women get killed by their partner every week. Tiny Man Tate has been a thing for years. Domestic violence shelters are overwhelmed. Women have been waving the red flags about incels and attitudes for years. We’ve been begging for change. We’ve been highlighting the serious threat of anti-women social media. Then a MAN makes a documentary and suddenly everyone’s interested? Do you not all think that this might be part of the problem?

@ken_trails @robertlufkinmd I think they are talking about protein sparing modified fast - so 500 to 900 calories per day of just lean protein, egg white, chicken, prawns that kind of thing. The idea is to get all the benefits of fasting without losing muscle mass.

One last beautiful push towards recruitment for my study. Eligibility criteria are as follows: Adults (18 years and older) or adolescents (ages 14-17) with signed parental consent. Must be using or have used a ketogenic diet as part of treatment for mental illness. Must have prior baseline and follow-up scores showing at least a 50% improvement on mood assessment (such as PHQ-9, GAD-7, DASS, PCL-5, or other validated instruments relevant to prior or existing diagnosis). Stable in the use of the ketogenic diet and not currently receiving regular consultation from a dietary professional. Self-reported confirmation of ketone levels at some point during the ketogenic diet treatment (through blood, breath, or urine). Currently, self-identify as recovered or in the process of recovering from a mental illness categorized under DSM-V. Martyna Ogonowska, a UK rape survivor who, at 18, stabbed a man to death in self-defense as he sexually assaulted her, has just lost an appeal of her 17 year prison sentence. This, lest we forget, is the same court system which failed to prosecute the adult man who raped Ogonowska when she was 14 because a judge decided that she’d consented, and which also recently handed out a suspended sentence to a man who raped a 13 year old because he told the judge that he “didn’t know it was illegal,” and because had been taught in his UK madrassah that British women and girls “have no more value than a piece of candy that has been dropped on the ground. It seems that the British justice system agrees.

There is a number attached to beef that gets cited at dinner parties. 15,000 litres per kilogram. Almonds: 3,500. Avocados: 2,000. Pistachios: 11,400. Beef, the headline says, uses more than all of them. Here is the thing nobody mentions at the dinner party. Water has categories. Green water: rainfall. Falls on a field, absorbed by vegetation, returned to the atmosphere. Recycled. Continuous. Not extracted from anything. The rain was going to fall on that land whether Gerald was on it or not. Attributing it to the animal standing in it is an accounting choice, not a physical reality. Blue water: groundwater and surface water. Pumped from rivers, drawn from aquifers. The water that, when it's gone, is gone. The water that is sinking the San Joaquin Valley. The number that matters. Gerald's blue water per kilogram of beef: approximately 50 litres. Californian almonds: 3,785 litres. Per kilogram. From an aquifer that took centuries to accumulate. The 15,000 litre figure is almost entirely green water. British rainfall, on British fields, falling regardless of Gerald. The dinner party number is the wrong number. The right number is 50 litres, and the almonds aren't close.

Hey Cheryl! Ohhh, that's such a hard question to answer...but honestly, the first thing that changed my life was hearing @drjasonfung plainly saying, "This is not your fault...you've been given bad advice for years!" I literally got tears in my eyes and felt the weight of shame lifting. So, I immediately took his advice to remove the remaining high-carb foods from my diet (I was still eating bread, potatoes, rice, and pasta) and stopped eating when I was NOT hungry. The subtitle of his book, The Obesity Code, is: Unlocking the Secrets of Weight Loss... And he did that for me! I'm so grateful to him and to God (I saw His hand over my journey step by step)!

Former fattie here, I agree with you wholeheartedly (the doctor and the need for proper diet) and I rarely listen to reels like this, but she looks so distressed, I did—and since I can relate to what she says, I can see some of her points and her reality. Is she completely honest with herself? NO. Neither was I. And I was given horrible advice for decades. @drjasonfung changed everything for me—finally sharing metabolic health wisdom I desperately needed. I do know and take full responsibility for my health (even if bad advice kept me in the obesity cycle for three decades). I am on a mission to help these women. I’m not even sure why I’m commenting, other than to say—I want this problem to be resolved 💔❤️‍🩹❤️ Freedom is possible. And honestly, the heavy weight of shame I carried was worse than the 100 extra pounds. #SugarFreed
