“The dose makes the poison” is a long accepted scientific axiom. One drop in a glass of water won’t kill you, 10 might, 100 definitely will. The same is true for masking. No mask stops 100% of particles from getting through. But the more we can lower the dose, the safer we are!

It’s wild that patients fear for their lives because their medical providers won’t practice basic public safety measures. And fate doesn’t have to be death. You can be terrified of a lifetime of chronic illness & medical complications. That’s absolutely terrifying.

Just had surgery. Many people are not masking. Saw an unmasked doctor coughing in post op. The unmasked charge nurse at this hospital just informed me that my having diabetes does not make me immunocompromised. This is @KeckMedicineUSC. We live in hell.

@dysclinic I was only patient at the infusion center who wore a mask during the three days I was there. One nurse wore a surgical mask. And my friend who visited wore N95. They asked me about Covid. There were signs everywhere about hand-washing. How are the pieces not fitting together?

Symptoms overlap but there are clear signs to help a provider encourage a patient to seek diagnosis. Took my rheum less than 2min: saw my patient history, my consistently flagged high HR & low BP, & asked if I ever saw stars when I stood. Then referred me to a POTS specialist.

Wish every headache specialist knew how to identify dysautonomia because migraine & POTS love to hold hands, esp after Covid. So many migraine patients w POTS will go for years before they receive a diagnosis, wondering if their weird symptoms are migraine or something else.

I was working this morning (due to an overflow of patients) and saw a woman in her early 40s who's had multiple medical and neurologic problems over the past few years. Aside from interstitial cystitis, her past medical history "appeared" unremarkable. In the middle of her health problems, she developed some type of twitching that a well-meaning neurologist at a good academic institution diagnosed as FND. The neurologist explained what FND was and how to treat it, but the patient didn't think that she had a software problem and never went back to that neurologist. The neurologist seemed dismissive of the burning pain in her legs, tachycardia, flushing and GI symptoms, but was adamant that the patient had FND. Complicating this story was the patient's current stress from a divorce, which was also rooted in the fact that the patient didn't feel well for years and was no longer able to keep up with lifestyle and fun activities that the spouse wanted to pursue. After several questions, I had no doubts that the patient had a hypermobility spectrum disorder (HSD), mast cell activation syndrome, #dysautonomia and other common complications of a connective tissue disorder. However, she has never heard these diagnoses from her doctors - after circulating in our healthcare system for years! Case in point: you cannot look at a patient with multisystemic symptoms with a "pick-and-choose" approach. If all you see is FND because the patient also had weird-looking twitching, but fail to see neuropathic pain, dysautonomia and connective tissue disorder that are at the core of the patient's suffering and disability, then I am sorry, but you're not doing a good job, and I don't care what specialty you come from. All specialists should learn how to recognize #HSD/#EDS and #Dysautonomia in their patients. #MedTwitter #NeuroTwitter #PatientCare #BrainHealth
